Instructions: Please complete the following form and submit. Leave any box blank if it doesn't apply. If you have already registered for the MM5 workshop, there is no additional fee. The deadline for registration, making payment and submitting the title for your presentation is May 1, 2000, and the deadline for submitting the extended abstract is June 2, 2000.

The workshop registration fee is $125.00. If you register after the deadline (May 1, 2000), the fee will be $175.00.
